Doing this up front saves time so our entire session can be about your needs, right now. After your completed form is received I will contact you to set-up a time for your session. If there are days of the week or times of day that work best, you can let me know in the last box on the form.
Since weight is not an accurate indicator of health you may leave those questions blank. (That feels better, doesn't it?)
Also, the information provided on these forms will only be used by us in our sessions. (So feel free to speak your truth!)
